提醒您 Notice	按下送出刪除註冊後，請務必至註冊電子郵件信箱收取確認信並完成後續作業，刪除健保卡網服務註冊申請始完成，感謝您的支持與配合。(若您已安裝【全民健保行動快易通】，完成確認刪除註冊後將無法再使用健康存摺及承保e櫃檯功能。) After submitting cancellation of the registration, please be sure to confirm that you receive a confirmation letter in your registration email box, and complete the following procedures to complete the cancellation. Thank you for your support and cooperation.(If you have installed NHI App, you will no longer be able to use My Health Bank and e-counter functions after confirming the cancellation.)

健保卡卡號 NHI card number	健保卡正面左下角12碼數字 The number on the bottom left side of the front of the NHI card.
E-Mail	因與微軟郵件伺服器連線異常，建議不要填寫微軟e-mail信箱(如hotmail, livemail...)，以免無法收到本署寄發的信件。 Due to the abnormal connection with the Microsoft mail server, it is recommended not to fill in the Microsoft Email address (such as hotmail, livemail ...), so as not to be able to receive the letter sent by NHIA.
戶籍鄉鎮里鄰 Household Registration Address	鄰(Neighborhood)
	正面 front 註：持有永久居留證者，【3居留期限】欄位請輸入申請當日日期。 For holders of permanent resident certificate, please enter the date of application on the “3. Date of expiry” field.
